Assignment/Change Form
   To be completed for new hires, changes in rate of pay, FTE changes, changes in employment locations on campus, changes in position or title, supervisor changes and for changes in benefit status.

All Performance Evaluations, including New Employee 90-day and 60-day reviews and yearly evaluations are to be completed electronically utilizing the PeopleAdmin system. For questions, please contact Human Resources. Paper forms are no longer accepted.

Taxes Name Change Governor's Executive Order Disclosures

NOTE:  As of 1/1/07, any new employee who was not employed with the university on January 1st, 2007 and had previous employment with the university, they must now complete a new hire packet upon re-employment.

NOTE:  A copy of your social security card and proper identification will also be required in order to add to Payroll. Please submit with New Hire Packet.

If you are hiring a part-time student worker who is international, they will need to complete all of the above forms and submit supporting identification that verifies identity and employment eligibility.  Acceptable documents are listed in Column A on the I-9, and include items such as a copy of an I-94 or the I-20.
